Flu Vaccine Information and Consent
Your child(ren) will each bring home a white envelope. This contains information about the Nasal Flu Vaccine together with a Consent Form.
The Vaccinations are due to take place in school on MONDAY 14th NOVEMBER 2022 from 1.00 pm onwards.
Please can you complete and return the Consent Forms by MONDAY 7th NOVEMBER at the latest. We need the forms returned even if you do not want your child to have the vaccine so we have a record of this.
